Why this applies to Amikacin
Amikacin falls under ANTIBIOTIC DRUGS — the group Bifidobacterium Longum interacts with.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: this interaction runs in one direction. Amikacin is a powerful antibiotic, and antibiotics don't care whether bacteria are harmful or friendly. Since Bifidobacterium longum is a live probiotic, amikacin can kill off a good chunk of those beneficial organisms, which makes the probiotic less likely to do its job. Your amikacin will still work just fine, so this is really about protecting the probiotic, not the medicine.
Keep in mind this concern is mostly theoretical, based on how the two work rather than hard studies. Also worth noting, amikacin is usually given as an injection or inhaled, so timing matters less than with a pill. Still, if you take both, talk with your pharmacist about spacing them out.

Arikayce is approved for adults with refractory Mycobacterium avium complex (MAC) lung disease — a serious bacterial lung infection that has not responded to at least 6 consecutive months of a standard multidrug antibiotic regimen. It is used as part of a combination treatment plan, not on its own.
This approval is limited: Arikayce is intended only for adults who have few or no other treatment options. It is not recommended for patients whose MAC lung disease has not yet failed standard therapy.
