Bifidobacterium Longum and Tobramycin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Tobramycin

Tobramycin falls under ANTIBIOTIC DRUGS — the group Bifidobacterium Longum interacts with.

Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to understand about pairing a probiotic like Bifidobacterium longum with tobramycin: the antibiotic doesn't get weaker, and your tobramycin will still work fine. It's the probiotic that takes the hit. Since these supplements contain live, friendly bacteria, an antibiotic that's busy killing bacteria can wipe out a good chunk of those helpful organisms too.

This is a theoretical concern based on how the drugs work, not a dangerous one, so don't stress about safety. The simple fix is timing: most pharmacists suggest spacing the two doses by at least 2 hours so the probiotic has a better shot at surviving. If you're using tobramycin for a serious infection, check in with your doctor or pharmacist about whether a probiotic makes sense for you right now.

From our pharmacist’s overview on the Tobramycin page

“Kidney protection is the first thing to understand with tobramycin (Nebcin, Tobi Podhaler, Bethkis, Kitabis PAK), an aminoglycoside antibiotic that can stress the kidneys even on its own, so this list warrants genuine care rather than a quick glance. The worst pairings, Siberian cocklebur and asarabacca, can injure the kidneys themselv…”
