Kidney protection is the first thing to understand with tobramycin (Nebcin, Tobi Podhaler, Bethkis, Kitabis PAK), an aminoglycoside antibiotic that can stress the kidneys even on its own, so this list warrants genuine care rather than a quick glance. The worst pairings, Siberian cocklebur and asarabacca, can injure the kidneys themselves, and adding them to tobramycin risks compounding that damage. Activated charcoal is the third serious entry; it can soak up many drugs and lower their levels in the body.

Most of the rest is the opposite problem: over half the entries are probiotic strains and fermented milk products that an antibiotic can simply kill off, meaning the supplement stops working, not the drug.
