Tobramycin falls under ANTIBIOTIC DRUGS — the group Limosilactobacillus Fermentum interacts with.

Theoretically, taking Limosilactobacillus fermentum with antibiotic drugs might decrease the effectiveness of L. fermentum.
Limosilactobacillus fermentum preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. fermentum preparations by at least two hours.

Here's the simple version: this interaction runs in one direction. Tobramycin is an antibiotic that kills bacteria, and the L. fermentum probiotic is itself a live, helpful bacteria. So if you take them at the same time, the tobramycin may kill off a good chunk of the probiotic before it can do its job. Your tobramycin keeps working just fine for your infection. It's the probiotic that loses some punch.

The good news is the fix is easy: just space them out, ideally by at least two hours. Keep in mind this concern is mostly theoretical, based on how antibiotics work, and it doesn't make your tobramycin any more dangerous. If you're on injected or inhaled tobramycin for a serious infection, check timing with your pharmacist or doctor first.

Tobramycin Injection is used to treat serious and life-threatening bacterial infections caused by susceptible bacteria. It is approved for adults and children for infections of the blood (septicemia), lungs and airways, brain and spinal cord (meningitis), abdomen, skin, bones, and urinary tract.
