Limosilactobacillus Fermentum and Bacampicillin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Bacampicillin

Bacampicillin falls under ANTIBIOTIC DRUGS — the group Limosilactobacillus Fermentum interacts with.

Theoretically, taking Limosilactobacillus fermentum with antibiotic drugs might decrease the effectiveness of L. fermentum.
Limosilactobacillus fermentum preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. fermentum preparations by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Bacampicillin is an antibiotic (it's actually a prodrug that your body converts into ampicillin once it's absorbed), and its whole job is to kill bacteria. The catch is that it can't tell the difference between the bacteria making you sick and the friendly probiotic bacteria in your L. fermentum supplement. So taking them at the same time can wipe out a good chunk of those live cultures before they ever get to work.

This won't make your antibiotic less safe or less effective, and it's mostly a theoretical concern based on how things work, not strong proof. To get more value from your probiotic, a common tip is to space them out by at least two hours. Check with your pharmacist if you're unsure about timing.

From our pharmacist’s overview on the Bacampicillin page

“If you're on bacampicillin (Penglobe), a penicillin-type antibiotic, handle this list with real care, starting with activated charcoal. It's the one major-rated interaction of the 44 on file, and it binds the antibiotic in your gut so less reaches your blood, which can mean an infection that doesn't clear.”
