Why this applies to Tobramycin
Tobramycin falls under ANTIBIOTIC DRUGS — the group Fermented Milk interacts with.
Some fermented milk preparations contain live and active organisms, including lactobacilli and bifidobacteria strains. Simultaneously taking antibiotics might kill a significant number of these organisms. Tell patients to separate administration of antibiotics and fermented milk preparations by at least 2 hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here the concern runs in one direction: the antibiotic affects the fermented milk, not the other way around. Tobramycin is a powerful germ-killer, and it does not tell the difference between the bacteria making you sick and the friendly probiotic strains (like lactobacilli and bifidobacteria) in your cultured milk. So taking them together may knock out those live cultures and waste the gut-health benefit you were hoping for.
The good news is this is theoretical, based on how the drug works rather than strong studies, and it does not make tobramycin less effective or more toxic. Tobramycin given as an injection works inside your body, while the probiotics live in your gut. To keep both happy, a simple fix is to space the fermented milk and any oral antibiotics by at least 2 hours. Your pharmacist can confirm what fits your situation.

Tobramycin Injection is used to treat serious and life-threatening bacterial infections caused by susceptible bacteria. It is approved for adults and children for infections of the blood (septicemia), lungs and airways, brain and spinal cord (meningitis), abdomen, skin, bones, and urinary tract.
Because antibiotic resistance is a growing concern, Tobramycin Injection should only be used when there is a proven or strongly suspected bacterial infection. Your care team will use culture and lab results to confirm it is the right choice.
