Neomycin falls under ANTIBIOTIC DRUGS — the group Fermented Milk interacts with.

Theoretically, antibiotic drugs might decrease the effectiveness of fermented milk.
Some fermented milk preparations contain live and active organisms, including lactobacilli and bifidobacteria strains. Simultaneously taking antibiotics might kill a significant number of these organisms. Tell patients to separate administration of antibiotics and fermented milk preparations by at least 2 hours.

Here's the thing to keep in mind: this isn't about the fermented milk hurting you or changing how Neomycin works. It's the other way around. Neomycin is an antibiotic, and antibiotics don't know the difference between bad bacteria and the friendly probiotic strains (like lactobacilli and bifidobacteria) living in a yogurt or kefir. Taking them at the same time can wipe out a good chunk of those live cultures, so you may not get the gut-health benefit you were hoping for from the fermented milk.

This is a theoretical concern based on how the drug works, not a dangerous one. A simple fix is to space them out by at least 2 hours. Neomycin itself will still do its job. If you have questions, your pharmacist is happy to help you sort out timing.
