AMINOGLYCOSIDE ANTIBIOTICS
Why this applies to Neomycin
Neomycin falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Magnesium interacts with.
Both aminoglycosides and magnesium reduce presynaptic acetylcholine release, which can lead to neuromuscular blockade and possible paralysis. This is most likely to occur with high doses of magnesium given intravenously.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with neomycin and magnesium. Both can quiet down the signal that nerves use to tell muscles to fire. Neomycin and magnesium each reduce the release of a messenger called acetylcholine at the nerve-muscle junction. Stack them together and, in theory, you could get more muscle weakness than usual.
Now for some reassurance. This concern really centers on high-dose magnesium given through an IV in a hospital, not the magnesium tablet or magnesium-containing antacid you take at home. The evidence here is anecdotal, so it's a possibility rather than something that happens often. Still, if you ever feel unusual weakness, trouble breathing, or floppy muscles, that's a call-your-doctor moment. It's always worth letting your pharmacist know everything you take.
