Here is the interaction we found between these two, with the evidence behind it.
ANTICOAGULANT/ANTIPLATELET DRUGS
Why this applies to Defibrotide Sodium
Defibrotide Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Magnesium interacts with.
In vitro evidence shows that magnesium sulfate inhibits platelet aggregation, even at low concentrations. Some preliminary clinical evidence shows that infusion of magnesium sulfate increases bleeding time by 48% and reduces platelet activity. However, other clinical research shows that magnesium does not affect platelet aggregation, although inhibition of platelet-dependent thrombosis can occur.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist if you're getting defibrotide (Defitelio). This is a powerful clot-preventing drug given by IV in the hospital after a stem-cell transplant, so it already nudges your blood toward less clotting and a bit more bleeding. Magnesium, in theory, may have its own mild blood-thinning effect by making platelets a little less sticky. Stacked together, the worry is slightly more bleeding, things like easy bruising, nosebleeds, or oozing.
That said, the evidence here is shaky and conflicting, and this is rated a minor and unlikely interaction. Honestly, since defibrotide is only given in a closely monitored hospital setting, your care team is already watching your bleeding risk carefully. Just make sure they know about any magnesium supplements you take so nothing slips through the cracks.

From our pharmacist’s overview on the Defibrotide Sodium page
“Defibrotide (Defitelio) is an intravenous drug for severe veno-occlusive disease of the liver after a stem cell transplant, and part of how it works is by reducing the blood's tendency to clot. That makes bleeding the single thread running through this entire list, and it deserves real care.”
