LEVODOPA/CARBIDOPA (Sinemet)

Magnesium can reduce the bioavailability of levodopa/carbidopa.
Clinical research in healthy volunteers shows that taking magnesium oxide 1000 mg with levodopa 100 mg/carbidopa 10 mg reduces the area under the curve (AUC) of levodopa by 35% and of carbidopa by 81%. In vitro and animal research shows that magnesium produces an alkaline environment in the digestive tract, which might lead to degradation and reduced bioavailability of levodopa/carbidopa.

Here's the practical concern with Stalevo, which combines levodopa, carbidopa, and entacapone. Magnesium, especially in concentrated supplement form like magnesium oxide, can make your stomach and gut more alkaline. That change can break down levodopa before your body absorbs it. In one study, a sizable magnesium dose cut the amount of levodopa reaching the bloodstream by about a third, and carbidopa dropped even more dramatically. For a Parkinson's medication, that matters, because less drug getting in can mean your symptoms (tremor, stiffness, slowness, or trouble moving) aren't as well controlled.

The good news is timing usually helps. Many people separate their magnesium and Stalevo by a couple of hours. Please check with your pharmacist or doctor before adjusting anything, especially if magnesium is treating a real deficiency.
