Treprostinil (Oral) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Magnesium interacts with.

Theoretically, magnesium may have antiplatelet effects, but the evidence is conflicting.
In vitro evidence shows that magnesium sulfate inhibits platelet aggregation, even at low concentrations. Some preliminary clinical evidence shows that infusion of magnesium sulfate increases bleeding time by 48% and reduces platelet activity. However, other clinical research shows that magnesium does not affect platelet aggregation, although inhibition of platelet-dependent thrombosis can occur.

Here's the deal with taking magnesium alongside oral treprostinil (Remodulin). Treprostinil itself works to relax and open up blood vessels in the lungs, and it can already make your blood platelets a little less sticky on its own. The thinking is that magnesium might also nudge platelets in that same direction, so in theory the two together could thin the blood slightly more than either alone.

I want to be honest though, the evidence here is weak and conflicting. Some lab and small studies hint at a mild effect, others show nothing. It's rated as a minor and unlikely interaction. For most people this isn't a big worry, but if you notice easy bruising, nosebleeds, or bleeding that won't stop, check in with your pharmacist or doctor.

About Treprostinil

What Is Treprostinil Used For?

Brand names: Remodulin, Orenitram

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
