Here is the interaction we found between these two, with the evidence behind it.
QUINOLONE ANTIBIOTICS
Why this applies to Levofloxacin
Levofloxacin falls under QUINOLONE ANTIBIOTICS — the group Magnesium interacts with.
Magnesium can form insoluble complexes with quinolones and decrease their absorption. Advise patients to take these drugs at least 2 hours before, or 4 to 6 hours after, magnesium supplements.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the deal with magnesium and levofloxacin: if you take them too close together, the magnesium can latch onto the antibiotic in your stomach and form clumps that your body can't absorb well. Less of the drug gets into your bloodstream, which means it may not fight your infection as strongly as it should. With antibiotics, you really want the full dose working, so a weaker effect could mean the infection lingers.
The good news is the fix is simple: spacing them out. A common suggestion is to take levofloxacin at least 2 hours before, or 4 to 6 hours after, any magnesium supplement or magnesium-containing antacid. This is well understood from how these drugs behave, so check with your pharmacist on timing before changing anything.
