LEVODOPA/CARBIDOPA (Sinemet)

Magnesium can reduce the bioavailability of levodopa/carbidopa.
Clinical research in healthy volunteers shows that taking magnesium oxide 1000 mg with levodopa 100 mg/carbidopa 10 mg reduces the area under the curve (AUC) of levodopa by 35% and of carbidopa by 81%. In vitro and animal research shows that magnesium produces an alkaline environment in the digestive tract, which might lead to degradation and reduced bioavailability of levodopa/carbidopa.

Here's the practical takeaway: magnesium can make your levodopa work less well. The reason is that magnesium (especially as magnesium oxide) makes the gut more alkaline, and levodopa breaks down in that environment before your body can absorb it. One study in healthy people found a 1000 mg magnesium oxide dose cut the amount of levodopa absorbed by about 35 percent, and the carbidopa by even more. So you're essentially getting a weaker dose.

For someone with Parkinson's, less absorbed levodopa can mean more off times, with stiffness, tremor, or trouble moving and walking. This is a real, well-documented concern. Please don't stop either one, but do talk with your pharmacist or doctor. Often, separating the timing or adjusting how you take them solves the problem.

What Is Levodopa Oral Inhalation Used For?

Brand names: Larodopa, Inbrija

Inbrija is used to treat OFF episodes in adults with Parkinson's disease. An OFF episode is when your regular oral carbidopa/levodopa medication temporarily stops working well enough, causing Parkinson's symptoms — such as stiffness, slowness, or tremor — to return.

Inbrija is used as needed when OFF symptoms start to come back. It is not a replacement for your regular oral carbidopa/levodopa; it works alongside it.
