AMINOGLYCOSIDE ANTIBIOTICS
Why this applies to Tobramycin
Tobramycin falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Magnesium interacts with.
Both aminoglycosides and magnesium reduce presynaptic acetylcholine release, which can lead to neuromuscular blockade and possible paralysis. This is most likely to occur with high doses of magnesium given intravenously.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for tobramycin: both this antibiotic and magnesium can quiet down the signal between your nerves and muscles. They each lower the release of a messenger chemical (acetylcholine) at that junction. Stack them together and you can get more muscle weakness than expected, and in rare, extreme cases trouble with breathing muscles.
The catch is that this is really only a concern with high-dose intravenous magnesium paired with injected tobramycin, usually in a hospital setting. An everyday oral magnesium supplement or magnesium in food is very unlikely to cause this. The evidence here is mostly anecdotal, so I don't want to alarm you. If you're getting tobramycin injections and also receiving IV magnesium, let your doctor or pharmacist know so they can watch for any new weakness.
