Plazomicin Sulfate falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Magnesium interacts with.

Concomitant use of aminoglycoside antibiotics and magnesium can increase the risk for neuromuscular weakness.
Both aminoglycosides and magnesium reduce presynaptic acetylcholine release, which can lead to neuromuscular blockade and possible paralysis. This is most likely to occur with high doses of magnesium given intravenously.

Here's the gist of pairing plazomicin (Zemdri) with magnesium. Both can quietly dampen the signal between your nerves and muscles by lowering the amount of a messenger chemical (acetylcholine) released. When you stack them, that effect can add up and lead to muscle weakness. In rare, more serious cases it could affect the muscles you use to breathe.

The good news: this is mostly a concern with high-dose magnesium given through an IV, not the magnesium tablets or antacids most folks take at home. The evidence here is pretty thin (mostly case reports), so don't panic. Since plazomicin is given in the hospital for serious infections, your care team is already watching for this. If you feel unusually weak or have any trouble breathing, tell them right away, and check with your pharmacist before adding magnesium supplements.

What Is Plazomicin Injection Used For?

Brand name: Zemdri

Zemdri (plazomicin) is approved for adults 18 and older to treat complicated urinary tract infections (cUTI), including pyelonephritis (a serious kidney infection), caused by certain susceptible bacteria such as Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Enterobacter cloacae.

Because clinical data on Zemdri are still limited, it is reserved for patients who have few or no other treatment options. It should only be used when the infection is confirmed or strongly suspected to be caused by bacteria that respond to it, in order to reduce the development of antibiotic-resistant bacteria.
