Magnesium and Streptomycin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

AMINOGLYCOSIDE ANTIBIOTICS

Why this applies to Streptomycin

Streptomycin falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Magnesium interacts with.

Concomitant use of aminoglycoside antibiotics and magnesium can increase the risk for neuromuscular weakness.
Both aminoglycosides and magnesium reduce presynaptic acetylcholine release, which can lead to neuromuscular blockade and possible paralysis. This is most likely to occur with high doses of magnesium given intravenously.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with streptomycin and magnesium. Both can quiet down the signal between your nerves and your muscles. They each slow the release of a chemical messenger called acetylcholine, and when you stack them together, that muscle-to-nerve signal can get a little too weak. The worry is added muscle weakness, and in rare, severe cases trouble with the muscles you use to breathe.

The good news: this is mostly a concern with high doses of magnesium given through the vein in a hospital, not the magnesium tablet you might take for sleep or constipation. The evidence here is fairly thin (mostly case reports), so don't panic. Still, since streptomycin is a serious injectable antibiotic, let your care team know about any magnesium you take so they can keep an eye on you.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

About Magnesium

What is Magnesium?

Magnesium
  • Preventing or treating magnesium deficiency
  • Constipation
  • Muscle cramps
  • Sleep support
  • Anxiety and stress
  • Migraine prevention

Magnesium is an essential mineral your body needs for muscles, nerves, blood pressure, and many other functions, and supplements are useful for preventing or correcting deficiency. Some other uses, like easing constipation or preventing migraines, have reasonable support, while uses for sleep and anxiety are less proven. It is generally safe for most healthy people at recommended amounts, but talk to your pharmacist or doctor before starting, especially if you have kidney problems.
