Neomycin falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the practical point with this pairing. When neomycin is taken by mouth (the oral tablets or liquid, often used to prep the bowel before surgery or for liver issues), it stays mostly in your gut and can interfere with how well things are absorbed there. The thyroid hormones in thyroid extract are absorbed in that same stretch of intestine, so taking the two close together may leave you absorbing less of the thyroid hormone than you'd expect.

This is rated minor and is mostly an issue of timing, not danger. The easy fix is separation: take your thyroid product on an empty stomach and space it well apart from oral neomycin. If you're using topical neomycin (a skin cream or ointment), this really doesn't apply. Since OTC thyroid extract doses aren't standardized anyway, please run your full routine past your pharmacist or doctor.
