Here the concern runs the other way from what folks usually expect. It is not that aprepitant (Emend, Cinvanti) changes your thyroid hormone, it is that taking an oral pill like aprepitant alongside thyroid extract can get in the way of how well the thyroid hormone is absorbed. When you swallow other oral medicines at the same time, they can blunt absorption of the thyroid hormone, so you may not get the full benefit of it.
The simple fix is timing, not stopping anything. Thyroid products work best on an empty stomach, well apart from other pills. Since aprepitant is used short-term around chemotherapy, this is usually a minor, manageable issue. Just ask your pharmacist how to space them out, and keep working with your doctor on any thyroid treatment.

Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. About Aprepitant
What Is Aprepitant Used For?
Brand names: Emend, Cinvanti
Aprepitant prevents nausea and vomiting. It is always used together with other anti-nausea medicines for chemotherapy.
Uses depend on the product. Emend (capsules and oral suspension) and aprepitant capsules are used for chemotherapy-related nausea and vomiting. Cinvanti (intravenous) is also for chemotherapy in adults. Aponvie (intravenous) and aprepitant capsules are used to prevent nausea and vomiting after surgery in adults. It has not been studied for treating nausea and vomiting that has already started.
