Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP3A4 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the gist for aprepitant (Emend, Cinvanti). Your body clears this medicine largely through a liver enzyme called CYP3A4. Some Acacia rigidula products have been found spiked with a stimulant called BMPEA, and in lab dish testing that compound slowed down CYP3A4 by about a third. If that enzyme works more slowly, aprepitant could hang around longer and build up, which might mean stronger effects and side effects like tiredness, hiccups, headache, or stomach upset.

I want to be honest about how shaky this is, though. This is only test-tube data, not real people, so it's theoretical. The bigger worry is that these supplements may contain a hidden stimulant at all. Please run any Acacia product past your pharmacist or oncology team first.

Acacia rigidula is a shrub from Texas and Mexico that is marketed in weight-loss and energy supplements. There is very little reliable human evidence that it works, and some products have been found to contain an unapproved stimulant drug (BMPEA), which raises real safety concerns. About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
