Aprepitant (Emend, Cinvanti) keeps chemotherapy patients from vomiting, so a supplement that undoes it mid-cycle is a problem, and I read this list with genuine vigilance. Nearly all 158 entries touch CYP3A4, the liver enzyme that clears aprepitant. Grapefruit slows the enzyme and can push levels too high, St. John's wort speeds it up and can drain the drug's protection, and activated charcoal soaks up an oral dose before it absorbs. Those three carry the major rating.
