Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.

Here's the situation with these two. Your body uses an enzyme called CYP3A4 to help clear aprepitant (Emend, Cinvanti). In test-tube studies, resveratrol can slow that same enzyme down. In theory, that could let aprepitant build up a little, which might mean more of its usual side effects like tiredness, hiccups, mild stomach upset, or headache.

I want to keep this in perspective, though. This concern comes from lab work, not real patients. In fact, one human study using a high resveratrol dose (3000 mg daily for 8 weeks) found no need to adjust CYP3A4 medications. So the real-world risk looks small. Aprepitant is also usually given for just a few days around chemo. Still, since you're going through chemotherapy, please run any resveratrol supplement past your oncology team or pharmacist first.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
