Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the simple version. Your liver uses an enzyme called CYP3A4 to break down modafinil, which is the active form your body makes after you take adrafinil. Aprepitant (Emend) mildly blocks that same enzyme. So in theory, while you're taking aprepitant, the modafinil from adrafinil could clear a little slower and stack up, leaving you with stronger stimulant effects than you expected. Think harder time sleeping, a racing or pounding heart, jitteriness, headache, or higher blood pressure.

To be fair, this is a theoretical concern based on how the drugs work, not something proven in studies, and aprepitant's effect on this enzyme is fairly modest and short-lived. Still, since adrafinil is unregulated and tough on the liver, please run it by your pharmacist or doctor before combining the two.

Here's the gist for Emend (aprepitant): your body clears it using a liver enzyme called CYP3A4. Adrafinil turns into modafinil in your liver, and modafinil can mildly speed up that same enzyme. In theory, that could clear aprepitant a little faster and leave less of it around to do its job.

Why that matters: aprepitant is given to prevent nausea and vomiting from chemotherapy. If its levels dip, it may not work as well, and the queasiness or vomiting it's meant to stop could break through. To be clear, this is a theoretical concern based on how these drugs behave, not something proven in studies, and the effect is thought to be weak. Still, since aprepitant's whole point is reliable nausea control during chemo, please run this past your oncology team or pharmacist before combining them.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
