Here's the thing with this pairing: Selpercatinib (Retevmo) can block an enzyme called CYP3A4, which is the same enzyme your liver uses to break down modafinil, the active form that adrafinil turns into. If that breakdown slows down, modafinil could build up a bit higher than expected. That might mean more of the wakefulness-related effects, like trouble sleeping, jitteriness, headache, a faster heartbeat, or feeling on edge.

I want to be honest about the strength of this, though. It's a theoretical concern based on how these drugs work, not something proven in studies. Still, since you're taking a serious cancer medicine, I wouldn't add an unregulated supplement like adrafinil (it also carries real liver risks) without checking with your oncologist or pharmacist first.

Here's the concern in plain terms. When your liver turns adrafinil into modafinil, that modafinil can gently speed up an enzyme called CYP3A4. Your cancer medicine, selpercatinib (Retevmo), is cleared by that same enzyme. So if the enzyme works a little faster, your body may clear the drug a bit quicker, which could lower its levels and possibly make it less effective at fighting your cancer. With a cancer treatment, you really want steady, reliable levels, so even a modest drop is worth taking seriously.

I'll be honest: this is a theoretical concern based on how these drugs behave, not proven harm. But because the stakes are high here, please don't add adrafinil without first checking with your oncologist or pharmacist.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
