Here's the gist of it. Adrafinil turns into modafinil in your body, and that modafinil is cleared by a liver enzyme called CYP3A4. Fluoxetine (Prozac) can slow that enzyme down a bit, so in theory the modafinil could linger longer and hit harder. That might mean more of the stimulant-type effects like trouble sleeping, jitteriness, a racing or pounding heart, headache, or feeling on edge.

I want to be honest that this is a theoretical concern, based on how the drugs work rather than real-world reports, so it may not amount to much for most people. Still, adrafinil is unregulated and rough on the liver, so please loop in your doctor or pharmacist before pairing it with fluoxetine.

Here's the idea behind this pairing. Adrafinil turns into modafinil in your body, and lab studies hint that modafinil may slow down a liver enzyme called CYP2C9. Since fluoxetine (Prozac) is partly handled by that same enzyme, the theory is that its levels could creep up a bit if the two are taken together.

If that happened, you might notice more of fluoxetine's usual effects, things like nausea, jitteriness, trouble sleeping, or headache. I want to be honest though: this is only a theoretical concern based on test-tube data, not real-world cases, so I wouldn't lose sleep over it. Adrafinil itself is unregulated and hard on the liver, so please check with your pharmacist or doctor before combining it with Prozac.

Fluoxetine is approved to treat four conditions: major depressive disorder (MDD), obsessive-compulsive disorder (OCD), bulimia nervosa (a binge-eating and purging disorder), and panic disorder — with or without agoraphobia (fear of situations that feel hard to escape).

It works for adults across all four conditions, and for children and teens with depression (ages 8–18) and OCD (ages 7–17).
