Here's the situation if you're taking Zoloft (sertraline) and thinking about adrafinil. Adrafinil is a prodrug that your liver turns into modafinil, and that modafinil is cleared by a liver enzyme called CYP3A4. Sertraline can mildly block that same enzyme. In theory, that means the modafinil could hang around a bit longer and build up, leading to stronger stimulant-type effects like trouble sleeping, a racing heart, jitteriness, headaches, or feeling on edge.

I'll be honest with you: this is a theoretical concern based on how the drugs work, not something proven in real patients, and sertraline is only a weak inhibitor of that enzyme. Still, adrafinil is unregulated and hard on the liver, so please check with your pharmacist or doctor before combining them.

Here's the gist for Zoloft (sertraline) with adrafinil. Once your liver turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4, which is one of the pathways that helps clear sertraline. In theory, that could nudge your sertraline levels a little lower over time, which might mean it doesn't control your mood, anxiety, or other symptoms quite as well.

I want to be honest though: this is a theoretical concern based on how these drugs work, not strong real-world data, and modafinil is only a weak booster of that enzyme. Sertraline also has other routes out of the body, so any change is likely small. Still, since adrafinil is unregulated and can stress the liver, please check with your doctor or pharmacist before combining them.

About Sertraline

What Is Sertraline Used For?

Brand name: Zoloft

Sertraline treats a range of mental health conditions. All formulations are approved for major depressive disorder (MDD) in adults and obsessive-compulsive disorder (OCD). The oral solution (including Zoloft oral solution) and several tablet formulations are also approved for panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).

For OCD specifically, the capsule formulations and some tablet products are approved for use in adults and children aged 6 and older. Sertraline hydrochloride tablets (plain, not film-coated) are approved for OCD in pediatric patients as well. However, sertraline is not approved for major depressive disorder in children — only in adults.
