Here's the gist for this combo pill (acetaminophen, caffeine, and dihydrocodeine). Adrafinil turns into modafinil in your body, and lab studies hint that modafinil may weakly speed up an enzyme called CYP1A2. That same enzyme helps clear caffeine, so in theory your caffeine could be broken down a bit faster. On its own that's pretty minor, and the caffeine in this product is mainly there to boost the pain relief.
Worth knowing: both adrafinil's modafinil and the caffeine are stimulants, so stacking them could leave you feeling jittery, anxious, restless, or unable to sleep. This interaction is only theoretical, not proven in people, so don't panic. Just check with your pharmacist or doctor before pairing them.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo pill. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. That same enzyme helps process the dihydrocodeine part of your pain medicine. Faster processing could mean the pain relief doesn't last as long or feels a little weaker, so you might notice more breakthrough discomfort.
I'll be honest, this is a theoretical concern based on how the drugs work, not something proven in real patients, and modafinil's effect on this enzyme is weak. The caffeine and acetaminophen pieces aren't a big deal here. Still, adrafinil is unregulated and hard on the liver, so please run this by your pharmacist or doctor before mixing the two.

Here's the thing to focus on with this combo product: it contains caffeine, which is a stimulant. Adrafinil turns into modafinil in your body, and modafinil is also a wakefulness booster. So taking them together can stack two stimulants on top of each other. That can leave you feeling jittery, anxious, or wired, with a faster heart rate, trouble sleeping, or a headache.
To be clear, this is a theoretical concern based on how these drugs work, not something proven in studies, so don't panic. The dihydrocodeine and acetaminophen parts aren't really the issue here.
