Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Aprepitant (Emend, Cinvanti). Your body relies on a liver enzyme called CYP3A4 to clear this medicine. Animal studies hint that a compound in Schizonepeta may speed up that enzyme. If that happens in people too, your body could break down aprepitant a bit faster, leaving less of it around to do its job.

Since aprepitant works as-is (it's not a prodrug that needs activating), faster clearance would mean a weaker anti-nausea effect, and the whole point of aprepitant is preventing the nausea and vomiting that chemo can cause. I'll be honest though: this is only theoretical, based on lab and animal data, not human studies. Still, this is a treatment you don't want to come up short on, so please check with your oncology pharmacist or doctor before adding Schizonepeta.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
