Roxithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the connection in plain terms. Once you swallow adrafinil, your liver turns it into modafinil, the active wakefulness drug. Modafinil is then cleared partly by a liver enzyme called CYP3A4. Roxithromycin, an antibiotic in the macrolide family, can mildly slow that same enzyme down.

So in theory, taking them together could let modafinil hang around a bit longer and feel stronger. That might mean more jitteriness, headache, trouble sleeping, a racing heart, or feeling on edge. I want to be honest though: this is a theoretical concern based on how the drugs work, not something proven in studies, and roxithromycin is only a weak blocker. Adrafinil itself is unregulated and hard on the liver, so please run this combination by your doctor or pharmacist first.
