Telithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the connection worth knowing. The wakefulness effect from adrafinil comes from your liver turning it into modafinil, and your body relies on an enzyme called CYP3A4 to clear that modafinil out. Telithromycin (Ketek) is a strong blocker of that same CYP3A4 enzyme. So while you're on this antibiotic, modafinil may break down more slowly and pile up a bit, which can mean stronger stimulant effects like jitteriness, a racing heart, trouble sleeping, headache, or higher blood pressure.

To be fair, this is theoretical, based on how the drugs work rather than real-world reports. Telithromycin is also only taken for a short course. Still, before pairing them, please check with your pharmacist or doctor.

Here's the short version: your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. Telithromycin (Ketek) is partly cleared by that same enzyme, so in theory, revving it up could lower telithromycin levels a bit. The concern is that a weaker level of antibiotic might not fight your pneumonia as well as it should.

I want to be honest about how solid this is. This warning is theoretical, based on how the drugs work on paper, not on real cases. Modafinil's effect on the enzyme is also fairly weak. Still, since you're treating a real infection, I'd loop in your doctor or pharmacist before combining them, especially given adrafinil's liver risks.

What Is Telithromycin Used For?

Brand name: Ketek

  • Telithromycin is used to treat certain types of pneumonia (an infection of the lungs) that is caused by bacteria. Telithromycin is in a class of medications called ketolide antibiotics. It works by killing bacteria.

  • Antibiotics such as telithromycin not work for colds, flu, or other viral infections. Taking antibiotics when they are not needed increases your risk of getting an infection later that resists antibiotic treatment.
