Bupivacaine, Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Zynrelef is a combination of bupivacaine (a local anesthetic) and meloxicam (an anti-inflammatory) placed right into a surgical wound. The meloxicam part is cleared by a liver enzyme called CYP2C9. Adrafinil, which your body turns into modafinil, may slow that enzyme down a bit. In theory, that could let meloxicam linger longer and raise its effects.
Honestly, this one is mostly on paper. The evidence is theoretical, from lab studies, and Zynrelef is a one-time dose given during surgery, so a meaningful interaction is unlikely. Still, higher meloxicam exposure could mean a bit more risk of stomach upset, bleeding, or kidney strain. Please mention any adrafinil use to your surgical team or pharmacist beforehand.
