Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture for Apadaz (benzhydrocodone with acetaminophen). Adrafinil turns into modafinil in your body, and modafinil may slightly speed up a liver enzyme called CYP1A2. The worry on file is that revving up this enzyme could lower the levels of drugs it handles. For your pain combo, though, this is very much a theoretical, lab-based concern, not something proven in real patients. CYP1A2 isn't the main pathway for either ingredient, so a big change is unlikely.
Still, if the two were ever taken together, keep an eye out for your pain pills feeling less effective. Don't adjust anything on your own. Check with your pharmacist or doctor first, especially since adrafinil is unregulated and can stress the liver.

Here's the thing to understand: adrafinil turns into modafinil in your body, and modafinil can gently speed up an enzyme called CYP3A4. Benzhydrocodone (the opioid part of Apadaz) is actually a prodrug that relies on your body to convert it into hydrocodone, the piece that relieves pain. If that conversion and clearing system runs a little faster, you could theoretically get less pain relief than expected, meaning your dose may not control your pain as well.
I'll be honest, this is a theoretical concern based on how the drugs work, not something proven in studies, and modafinil's effect on this enzyme is weak. Still, if your pain isn't being managed, don't adjust anything on your own. Loop in your doctor or pharmacist first.

What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
