The piece of this combo pill that matters here is the caffeine. Your liver clears caffeine using an enzyme called CYP1A2, and adrafinil (which your body turns into modafinil) may speed that enzyme up a little. In theory, that means caffeine could be cleared a bit faster, so you might notice less of its pick-me-up effect.

Honestly, this is a theoretical concern based on lab data only, and the effect on caffeine looks weak, so it's nothing to panic over. The aspirin and dihydrocodeine parts aren't really affected through this pathway. My bigger flag is that adrafinil itself is unregulated and can stress your liver. Please run this by your doctor or pharmacist before combining them.

Here's the thing to know about this combo product (Synalgos DC contains aspirin, caffeine, and dihydrocodeine). When your body turns adrafinil into modafinil, modafinil can mildly speed up a liver enzyme called CYP3A4. Several of these ingredients pass through that same pathway, so in theory they could be cleared a little faster than usual.

Dihydrocodeine is a prodrug, meaning your liver has to activate it to get full pain relief, so the direction here isn't simple, and the practical effect is likely small. Still, you might notice slightly weaker pain control. I'd flag this for your pharmacist before combining them, especially since adrafinil is unregulated and rough on the liver.

The piece of Synalgos-DC that matters here is the caffeine. Once your liver turns adrafinil into modafinil, you essentially have a wakefulness booster on board. Stack that on top of the caffeine already in this pain combo and you can get an additive stimulant effect, meaning more jitteriness, a racing or pounding heartbeat, trouble sleeping, anxiety, or feeling wired.

To be honest, this is a theoretical concern based on how these compounds work, not something proven in solid studies, so the real-world risk is hard to pin down. It does not change how the aspirin or the dihydrocodeine (the opioid pain piece) behaves. Adrafinil is also unregulated and hard on the liver, so please run this combination by your pharmacist or doctor before mixing them.
