Adrafinil and Chlorpheniramine, Pheylephrine, Pyrilamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Chlorpheniramine, Pheylephrine, Pyrilamine

Chlorpheniramine, Pheylephrine, Pyrilamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with this combo. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up a liver enzyme called CYP3A4. Some of the ingredients in Phena-Plus pass through that same enzyme, so in theory they could be cleared a little faster and feel a bit weaker. In plain terms, your allergy and congestion relief (the runny nose, sneezing, and stuffiness help from the chlorpheniramine, phenylephrine, and pyrilamine) might not work quite as well.

The good news: this is a theoretical concern based on how the drugs behave, not strong real-world reports, and the enzyme effect is weak. I wouldn't lose sleep over it. Still, since adrafinil is unregulated and hard on the liver, run it by your pharmacist or doctor first.
