Adrafinil and Ethinyl Estradiol, Norgestimate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Adrafinil interacts with whole categories of medication. Ethinyl Estradiol, Norgestimate falls into 3 of those categories, so it can interact with Adrafinil in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Ethinyl Estradiol, Norgestimate

Ethinyl Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with your birth control pill (Ortho Tri-Cyclen and similar combos). Adrafinil turns into modafinil in your body, and modafinil can gently speed up a liver enzyme called CYP1A2. The worry is that revving up liver activity could lower the levels of the hormones in your pill, which in theory might make it a little less reliable at preventing pregnancy. You could also notice breakthrough bleeding or spotting.

I want to be honest, though: this particular concern is theoretical, based on lab data rather than real patient cases, so the actual risk is uncertain. Even so, since an unplanned pregnancy is a big deal, I'd run this by your doctor or pharmacist before combining them. Please don't stop your pill on your own.

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

2 of 3

Ethinyl Estradiol, Norgestimate also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the thing to keep in mind with your birth control pill (Ortho Tri-Cyclen and similar combos contain ethinyl estradiol with norgestimate). On paper, these hormones can mildly slow down the CYP3A4 enzyme, which is the same liver pathway that turns adrafinil into its active form, modafinil. So in theory, your pill could let a little more modafinil build up, meaning the wakefulness effects (and side effects like jitteriness, headache, trouble sleeping, or a racing heart) might feel stronger.

This is theoretical only, based on how the drugs work, not on real-world reports. What worries me more is the flip side: modafinil itself can speed up hormone breakdown and make birth control less reliable. Please talk with your pharmacist or doctor before combining these, and ask about backup contraception.

Here's the thing to know if you're on Ortho Tri-Cyclen, Sprintec, or a similar birth control pill and thinking about adrafinil. Your body turns adrafinil into modafinil, and modafinil can mildly speed up a liver enzyme called CYP3A4. That same enzyme helps break down the estrogen and progestin hormones in your pill. So in theory, those hormones could clear out a little faster, which may lower how well the pill prevents pregnancy and can lead to breakthrough bleeding or spotting.

I'll be honest, this is based on how the drugs work, not on solid studies, and the effect is thought to be weak. Still, an unplanned pregnancy is a big deal, so please talk with your doctor or pharmacist about a backup method (like condoms) before mixing these.
