Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Prevpac is a combination pack used to clear an H. pylori stomach infection. The piece that matters here is lansoprazole, the acid reducer. Your liver uses an enzyme called CYP2C19 to break it down. Adrafinil turns into modafinil in your body, and modafinil can mildly slow that same enzyme. In theory, that could let a bit more lansoprazole build up, which might mean slightly stronger acid suppression or more of its usual minor effects like headache, diarrhea, or stomach upset.
Honestly, this is a theoretical concern based on lab data, not real patient reports, so I wouldn't lose sleep over it. Since lansoprazole is usually a short course, the risk is low. Still, run any adrafinil use by your pharmacist or doctor first, especially given its liver risks.

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Prevpac is a combo pack used to treat H. pylori stomach infections, and one of its ingredients, clarithromycin, is a fairly strong blocker of a liver enzyme called CYP3A4. That same enzyme is what turns adrafinil into active modafinil and then helps clear it out. So in theory, taking clarithromycin alongside adrafinil could let modafinil build up a bit more than usual.
What that might feel like is stronger stimulant effects: trouble sleeping, jitteriness, a racing heart, headache, or anxiety. I'll be honest though, this is a theoretical concern based on how the drugs work, not something proven in studies. Adrafinil is also unregulated and hard on the liver, so before combining them, please check in with your doctor or pharmacist.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Prevpac is a three-drug combo (lansoprazole, amoxicillin, and clarithromycin) used together to wipe out H. pylori stomach infections and heal ulcers. The piece that matters here is clarithromycin, which the liver processes through an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme.
In theory, that faster clearance could lower clarithromycin levels a bit. With an antibiotic, even small drops are worth watching, since you want enough drug on board to fully clear the infection and avoid treatment failure. To be clear, this is a theoretical concern based on pharmacology, not strong real-world data, and the effect is considered weak. Still, please run it by your pharmacist or doctor before combining them.
