Adrafinil + Acetaminophen, Aspirin, Caffeine
Acetaminophen, Aspirin, Caffeine brand names: Excedrin Migraine, Excedrin, Excedrin Extra Strength.
Acetaminophen, Aspirin, Caffeine falls into 3 drug categories that Adrafinil can interact with; the most serious is rated moderate. Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Adrafinil interacts with whole categories of medication. Acetaminophen, Aspirin, Caffeine falls into 3 of those categories, so it can interact with Adrafinil in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Aspirin, Caffeine
Acetaminophen, Aspirin, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Excedrin combo (acetaminophen, aspirin, and caffeine). One of these three, the caffeine, is mostly broken down by a liver enzyme called CYP1A2. Modafinil, which your body makes from adrafinil, may speed that enzyme up a little. In theory, that could clear caffeine a bit faster, meaning the caffeine in your Excedrin might feel slightly weaker or wear off sooner. The acetaminophen and aspirin pieces aren't really affected this way.
I want to be honest that this is theoretical, based only on lab data, not real-world reports, so I wouldn't lose sleep over it (no pun intended). Both adrafinil and caffeine are stimulants, though, so stacking them could leave you feeling jittery or wired. Please run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Excedrin (acetaminophen, aspirin, and caffeine combo). Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up a liver enzyme called CYP3A4. Since this enzyme helps process part of the acetaminophen in Excedrin, the theory is that you might clear it a little faster, which could slightly soften the pain or headache relief you're after.
I want to be honest though: this is a theoretical concern based on how the drugs work, not something proven in real patients, and modafinil only nudges that enzyme weakly. For an occasional headache dose it's unlikely to be a big deal. Just loop in your pharmacist or doctor before pairing adrafinil with anything, since it carries its own liver risks.

STIMULANT DRUGS
3 of 3Acetaminophen, Aspirin, Caffeine also falls under STIMULANT DRUGS.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this combo: the part that matters is the caffeine in Excedrin Migraine. Adrafinil turns into modafinil in your liver, and modafinil has stimulant-like, wake-you-up effects. Stack that on top of caffeine and the two can add together.
In plain terms, you might feel more jittery, have a racing or pounding heart, trouble sleeping, anxiety, or a touch of nausea. This is based on how these compounds work rather than on solid studies, so it's more of a theoretical concern than a proven one. The acetaminophen and aspirin pieces aren't really the issue here. If you use adrafinil, it's worth a quick chat with your pharmacist or doctor first, especially since adrafinil itself can stress the liver.
