These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Adrafinil interacts with whole categories of medication. Chlorpheniramine, Hydrocodone, Pseudoephedrine falls into 2 of those categories, so it can interact with Adrafinil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Chlorpheniramine, Hydrocodone, Pseudoephedrine
Chlorpheniramine, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Tussend, which combines hydrocodone (a cough and pain opioid), chlorpheniramine (an antihistamine), and pseudoephedrine (a decongestant). Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Since hydrocodone is partly handled by CYP3A4, in theory your body could clear it a little faster, which might mean weaker pain or cough relief from this product.
I want to be honest: this is a theoretical concern based on how the drugs work, not something proven in real patients, and modafinil's effect on this enzyme is weak. Adrafinil itself is unregulated and tough on the liver, so I'd run this combo by your pharmacist or doctor before mixing them rather than changing anything on your own.

STIMULANT DRUGS
2 of 2Chlorpheniramine, Hydrocodone, Pseudoephedrine also falls under STIMULANT DRUGS.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters in this combo: your Tussend product contains pseudoephedrine, which is itself a stimulant decongestant. Adrafinil turns into modafinil in your body, and modafinil also has wakefulness-promoting, stimulant-like effects. So when you stack the two, the worry is that those stimulant effects add up. You might notice a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, or feeling anxious or wired.
This is a theory-based caution drawn from how these compounds work, not from strong studies, so it isn't a guarantee it'll happen. Still, the hydrocodone in Tussend can make you drowsy, and mixing a sedating opioid with two stimulants is a confusing tug-of-war for your body. Please check with your pharmacist or doctor before combining them.
