Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Tansy Ragwort interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in tansy ragwort.
Tansy ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

First things first: I'd steer clear of tansy ragwort altogether. It contains pyrrolizidine alkaloids that can poison the liver, and there's no good reason to take it. Aprepitant (Emend, Cinvanti) is your anti-nausea medicine for chemo, and it actually nudges up the activity of a liver enzyme called CYP3A4.

Here's the theoretical worry: that same enzyme turns tansy ragwort's alkaloids into toxic byproducts. So in theory, aprepitant could speed up that conversion and raise the odds of liver harm. I want to be honest, though, this concern comes only from lab and animal data, not from real cases in people. The real takeaway is simpler: don't use tansy ragwort, and please mention any herbal products to your oncology team or pharmacist.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
