Enzalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Tansy Ragwort interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in tansy ragwort.
Tansy ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

First things first: tansy ragwort really shouldn't be taken internally at all. It contains pyrrolizidine alkaloids, which can poison the liver even on their own. So my honest advice is to skip the tansy ragwort entirely, regardless of your other medications.

Here's the specific concern with Xtandi (enzalutamide): it speeds up a liver enzyme called CYP3A4. That same enzyme is what turns tansy ragwort's alkaloids into harmful, liver-damaging byproducts. So in theory, enzalutamide could ramp up that toxic conversion and raise the risk of liver injury. I'll be straight with you, this is a theoretical worry based on lab data, not real patient cases. But since tansy ragwort is dangerous anyway, please talk to your oncologist or pharmacist and avoid it.

About Enzalutamide

What Is Enzalutamide Used For?

Brand name: Xtandi

Xtandi (enzalutamide) is used to treat prostate cancer in three specific situations: castration-resistant prostate cancer (CRPC), metastatic castration-sensitive prostate cancer (mCSPC), and non-metastatic castration-sensitive prostate cancer with biochemical recurrence at high risk for metastasis (high-risk BCR).

Castration-resistant means the cancer has continued to grow despite low testosterone levels. Castration-sensitive means the cancer still responds to testosterone-lowering therapy. In most of these cases, Xtandi is used alongside a GnRH analog (a hormone-lowering injection) or after surgical removal of the testicles — except in the high-risk BCR setting, where it may be used with or without a GnRH analog.
