Telotristat Etiprate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Tansy Ragwort interacts with.
Tansy ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Let me be upfront: tansy ragwort isn't something you should be taking at all. It contains pyrrolizidine alkaloids, plant compounds that your liver can turn into toxic substances that damage liver cells. There's no good evidence it treats anything, and the real concern here is liver harm, not whether it helps your carcinoid symptoms.
Telotristat (Xermelo) can act as a mild CYP3A4 inducer, the same liver pathway that converts those alkaloids into their harmful forms. So in theory, taking the two together could speed up the production of the toxic byproducts and add to the liver risk. I'll stress that this is theoretical, based on lab work rather than real patient cases. Still, the safest move is simple: skip the tansy ragwort, and check with your pharmacist or doctor before any herbal product.

Xermelo (telotristat) tablets are approved to treat carcinoid syndrome diarrhea in adults. Carcinoid syndrome is a set of symptoms caused by certain tumors — called neuroendocrine tumors — that release excess serotonin into the body, triggering frequent, often severe diarrhea.
Xermelo is used in combination with somatostatin analog (SSA) therapy — a class of injected medications commonly used to control carcinoid syndrome — in patients whose diarrhea is still not adequately controlled by SSA therapy alone.
