Why this applies to Repotrectinib
Repotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Tansy Ragwort interacts with.
Tansy ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
First things first: tansy ragwort really shouldn't be taken by mouth at all. It contains pyrrolizidine alkaloids, which are known to harm the liver, and there's no good evidence it helps any condition. So my honest advice is to steer clear of it regardless of what medicine you're on.
Here's the specific concern with Repotrectinib (Augtyro). This cancer drug is a mild-to-moderate CYP3A4 inducer, meaning it can rev up that liver enzyme. The tansy ragwort toxins are activated by that same enzyme, so in theory a stronger enzyme could turn more of them into liver-damaging byproducts. This is only a theoretical, lab-based idea, not something proven in people, so I don't want to alarm you. Please just don't use tansy ragwort, and check with your oncology team or pharmacist before adding any herbal product.
