Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Tansy Ragwort interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in tansy ragwort.
Tansy ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the short version: I'd steer clear of tansy ragwort entirely, and not just because of Lorbrena. This plant contains pyrrolizidine alkaloids (PAs) that can seriously harm your liver on their own, so it isn't safe as a home remedy in the first place.

The specific concern here is that lorlatinib can speed up an enzyme called CYP3A4. That same enzyme turns the plant's PAs into toxic byproducts, so in theory lorlatinib could increase the damage those compounds do to your liver. I want to be honest that this is a theoretical worry based on lab data, not human studies. Still, since lorlatinib itself can already raise liver enzymes, stacking another liver stressor isn't worth it. Please skip tansy ragwort and check with your oncology pharmacist before any herbal product.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
