Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Tansy Ragwort interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in tansy ragwort.
Tansy ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

First things first: tansy ragwort really shouldn't be taken internally at all. It contains pyrrolizidine alkaloids, which your liver can turn into substances that harm liver cells and the small blood vessels inside the liver. So the bigger concern here is the plant itself, not the medicine.

Now, the specific interaction. Fosaprepitant (Emend) is given before chemotherapy to prevent nausea and vomiting, and it can act as a mild CYP3A4 inducer, meaning it can speed up that liver enzyme. In theory, that faster activity could ramp up the conversion of tansy ragwort's compounds into the toxic versions, raising the risk of liver injury. I want to be honest: this is theoretical, based on lab thinking rather than human studies. Even so, the safest move is to simply avoid tansy ragwort and check with your pharmacist or doctor.
