Acetaminophen, Caffeine, Codeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill has three pieces, and adrafinil (once your liver turns it into modafinil) may gently speed up an enzyme called CYP1A2. The piece most affected is the caffeine. If that enzyme runs a little faster, caffeine could clear out sooner, so you might notice it wears off quicker or feels weaker. That said, you're also stacking two stimulant-like effects here, so jitteriness, a racing heart, trouble sleeping, or anxiety could go the other way.
Honestly, this is a theoretical concern based on lab data, not strong real-world proof, so I wouldn't panic. The acetaminophen and codeine parts aren't really driven by CYP1A2. Adrafinil itself is unregulated and hard on the liver, so please run this past your pharmacist or doctor before mixing them.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo pill (acetaminophen, caffeine, and codeine). Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP3A4. That same enzyme helps clear caffeine and parts of this pain reliever, so in theory adrafinil could lower their levels a bit. Keep in mind this is only a theoretical concern based on how the drugs work, not from strong studies, so it may never cause a noticeable problem.
One wrinkle: codeine is a prodrug that your body must convert (mostly through a different enzyme, CYP2D6) into the active morphine that relieves pain. Because the main effect here is on CYP3A4, the pain relief itself probably isn't changed much. If you take both regularly, just run it by your pharmacist or doctor to be safe.

This combination pill has three ingredients, but the one that matters here is the caffeine. Adrafinil turns into modafinil in your liver, and modafinil works like a stimulant. Stack that on top of the caffeine in this product and the two can add up, giving you more of the jittery, wired feeling you might already get from caffeine: a racing or pounding heart, trouble sleeping, anxiety, or feeling shaky.
To be honest, this is a theoretical concern based on how these compounds behave, not a proven, dangerous reaction, so I don't want to alarm you. Still, since adrafinil is unregulated and hard on the liver, please run it by your pharmacist or doctor before pairing it with anything, including this medication.
