Here's the situation with Fintepla (fenfluramine) and adrafinil. Adrafinil turns into modafinil in your body, and modafinil may gently speed up a liver enzyme called CYP1A2, which is one of the pathways that helps break fenfluramine down. If that enzyme works faster, your body could clear fenfluramine a little quicker, which in theory might make it slightly less effective at keeping seizures under control.

I want to be honest about how soft this evidence is. It comes from lab studies only (not real patients), and the enzyme nudge is described as weak, so the real-world risk is probably small. Still, fenfluramine is a seizure medicine where steady levels matter, and adrafinil carries its own liver risks. Please check with your doctor or pharmacist before combining them.

Here's the gist for Fintepla (fenfluramine). Adrafinil turns into modafinil in your body, and lab studies hint that modafinil can mildly speed up an enzyme called CYP2B6, which is one of the routes your body uses to clear fenfluramine. In theory, that faster clearance could lower your fenfluramine levels and make it work less well, meaning seizures might not be as well controlled.

I want to be honest though: this is a theoretical concern based on test-tube data, not real patient cases, so we don't know if it truly matters. But seizure control is serious business, and adrafinil is unregulated with real liver risks. Please loop in your child's neurologist or pharmacist before combining these.

What Is Fenfluramine Used For?

Brand name: Fintepla

Fintepla (fenfluramine oral solution) is approved to treat seizures associated with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It is used alongside other antiseizure medications.

Both conditions are severe, often hard-to-control forms of epilepsy that typically begin in childhood. Fintepla is available only through a restricted program called the Fintepla REMS because of serious heart-related risks.
