Here's the gist: once your body turns adrafinil into modafinil, that modafinil can slow down a liver enzyme called CYP2C19. Imipramine partly relies on that same enzyme to be cleared from your system. So in theory, pairing the two could let imipramine build up a bit more than usual.

If levels climb, you might notice more of imipramine's typical effects, things like drowsiness, dry mouth, constipation, blurred vision, dizziness when standing, or a faster heartbeat. I want to be honest, though: this is theoretical, based on lab studies rather than real patients, so it may not amount to much. Still, since tricyclics can be touchy, please run this combination by your pharmacist or doctor before mixing them.

Here's the gist for imipramine (Tofranil). Once your liver turns adrafinil into modafinil, that modafinil may slightly speed up an enzyme called CYP1A2, which is one of the pathways your body uses to clear imipramine. In theory, that could nudge your imipramine levels down a bit, which might make it work less well. For depression that could look like your symptoms creeping back, and for a child using it for bedwetting, you might notice it isn't holding as well.

I want to be honest about how soft this evidence is. This is a theoretical concern based mostly on lab data, and the enzyme effect is described as weak. It's not a strong or guaranteed interaction. Still, since these are very different medicines, please loop in your doctor or pharmacist before combining them.

Once your liver turns adrafinil into modafinil, that modafinil may slow down a liver enzyme called CYP2C9, which is one of the helpers that breaks imipramine down. If that breakdown slows, imipramine could build up a bit and its effects, good and bad, may get stronger.

With a tricyclic like imipramine, that could mean more drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat. Here's the gist: once your liver turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4. Imipramine is partly broken down by that same enzyme, so in theory your body could clear it a little faster, which might leave slightly less of the drug working in your system. For depression, that could mean your antidepressant effect feels a touch weaker over time.

I want to be honest with you though: this is a theoretical concern based on how these drugs behave, not solid real-world data, and modafinil is only a weak enzyme nudger. Adrafinil also stresses the liver on its own. If you take Imipramine, please loop in your doctor or pharmacist before adding adrafinil so they can watch your mood and dose.

About Imipramine

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
