Cyclophosphamide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.

Here's something worth knowing if you're on cyclophosphamide (Cytoxan). This chemo drug is actually a prodrug, meaning your liver has to switch it on before it can fight cancer. One of the enzymes that helps activate it is CYP2C19. Adrafinil turns into modafinil in your body, and modafinil can slow down that same enzyme, at least in lab studies.

So in theory, blocking CYP2C19 could mean less of the active, cancer-fighting form being made, which might make your treatment work less well. I'll be honest, this is a theoretical concern based on pharmacology, not proven in patients. Still, with a drug this important, you don't want to gamble. Please loop in your oncologist or pharmacist before mixing the two.

Here's the wrinkle with this pair. Cyclophosphamide is actually a prodrug, meaning it doesn't do much until your liver (largely through the CYP3A4 and CYP2B6 enzymes) converts it into its active, cancer-fighting form. Adrafinil turns into modafinil in your body, and modafinil can weakly speed up CYP3A4. So in theory, this could shift how cyclophosphamide is processed.

I want to be honest that this is theoretical, based on how the drugs work rather than solid studies in people, so I wouldn't lose sleep over it. Still, cyclophosphamide is serious chemotherapy with real risks (low blood counts, infection, bladder irritation), and you don't want any guesswork around its dosing. Please don't add adrafinil without first running it by your oncology team or pharmacist.

About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
