Here's the gist: clarithromycin (Biaxin) is a strong blocker of a liver enzyme called CYP3A4, and that same enzyme helps clear modafinil, which is what adrafinil turns into in your body. So if you take both, the modafinil could hang around longer and build up. That may mean stronger stimulant-type effects, things like trouble sleeping, jitteriness, a racing heart, headache, or feeling anxious.

I want to be honest, though: this concern is theoretical, based on how the drugs work on paper rather than real cases in people. Still, adrafinil is unregulated and hard on the liver, so I'd play it safe. Please check with your doctor or pharmacist before pairing these two.

Here's the idea: your liver turns adrafinil into modafinil, and modafinil can gently speed up an enzyme called CYP3A4. That same enzyme helps clear clarithromycin from your body. If it works a little faster, your clarithromycin levels could dip slightly, which in theory might make the antibiotic less effective against the infection you're treating.

I want to be honest about how soft this is. It's a theoretical concern based on how these drugs behave, not on real patient cases, and modafinil is only a weak nudge on that enzyme. Still, you want an antibiotic working at full strength. Since adrafinil is unregulated and hard on the liver anyway, please run this combination by your pharmacist or doctor before mixing them.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
