Acetaminophen, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with your Sinutab or Tylenol Sinus product paired with adrafinil. Once your liver turns adrafinil into modafinil, that modafinil may gently speed up an enzyme called CYP1A2. In theory, a faster enzyme could clear certain medications a bit quicker than usual.
The good news: this is a very weak, theoretical concern based only on lab studies, not real patient reports. The acetaminophen and pseudoephedrine in these cold products aren't strongly dependent on CYP1A2, so a meaningful change in your fever, pain, or congestion relief is unlikely. I wouldn't lose sleep over it. Still, adrafinil itself carries real liver and heart risks, so please check in with your pharmacist or doctor before combining the two.

Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece to focus on here is the pseudoephedrine, which is the decongestant in your Sinus product. It's a stimulant that can already speed up your heart and raise your blood pressure. Adrafinil turns into modafinil in your body, and modafinil also has a stimulating, wake-you-up effect. Stacking the two could add up, so you might notice a racing or pounding heart, jitteriness, anxiety, higher blood pressure, or trouble sleeping. The acetaminophen part isn't really involved in this concern.
To be fair, this is a theoretical caution based on how these compounds work, not strong real-world data. Still, if you're considering using both, please check with your pharmacist or doctor first, especially if you have any heart or blood pressure issues.
