Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist with aprepitant (Emend, Cinvanti) and milk thistle. Aprepitant is cleared by a liver enzyme called CYP3A4, and milk thistle might slow that enzyme down a little. In theory, if it did, your body could hold onto aprepitant a bit longer, which could mean more of its usual effects like tiredness, hiccups, mild stomach upset, or headache.

The honest truth is the evidence here is weak. Careful human studies found milk thistle did not meaningfully change how similar CYP3A4 drugs were broken down, and this concern leans on lab tests and a few scattered case reports. So this is a minor, unlikely interaction. Still, since you're going through chemo, it's worth a quick chat with your oncology pharmacist or doctor before adding milk thistle.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
